Once completed, please print, sign, and fax the form or submit it as a scanned image, together with a copy of your passport or ID to AL Trade Inc., using the fax number or e-mail address indicated at the bottom the form.
   
   After receiving your request, a completed changes confirmation for your trading account will be sent to your e-mail. 
    

3. Contact information change request

   
   In order to change your AL Trade Inc. account information you need to open this form. Please carefully complete this on-line form, indicating the appropriate fields, and confirm it.
